Waiver of Liability

I hereby acknowledge and agree that I have requested cleaning services from Lexie’s Sparkle & Shine Cleaning Services and thus I understand and voluntarily accept the following terms and conditions:

• Assumption of Risks: I understand that cleaning services involve certain risks and hazards including but not limited to slips, falls, allergic reactions, or damage to property. I voluntarily assume all risks associated with the cleaning services provided by Lexie’s Sparkle & Shine Cleaning Services.

• Release of Liability: I hereby release, waive, discharge, and covenant not to sue Lexie’s Sparkle & Shine Cleaning Services, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or harm that may occur as a result of the cleaning services provided.

• Indemnification: I agree to indemnify and hold harmless Lexie’s Sparkle & Shine Cleaning Services, its owners, employees, agents, and representatives from any and all claims, demands, liabilities, damages, or expenses (including attorney's fees) arising out of or related to my use of the cleaning services.

• Acknowledgment of Understanding: I acknowledge that I have read this waiver of liability, fully understand its terms and conditions, and agree to be legally bound by them. I understand that by signing this waiver, I am giving up certain legal rights and remedies that I may otherwise have.

• Severability: If any provision of this waiver of liability is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I hereby acknowledge that I have read and understand the terms of this waiver of liability and voluntarily agree to its provisions.

These terms and conditions are within the jurisdiction of the State of New York.
